Grant Program Information
The Portland IPA offers grant funding to support PIPA network primary care and specialist network providers in developing and implementing innovative projects that improve the quality or efficiency of patient care. Projects submitted within the grant timeline will be evaluated based on the criteria listed below.
Key Dates for the 2024 Grant Program
- Application process opens: October 20, 2023
- Application closes: December 1, 2023
- Notification of final decisions: by December 31, 2023
- Final reports due: by December 27, 2024
- Grant Event for 2024 projects – Spring of 2025
- Grant Event for 2023 projects – Spring of 2024 (more information to come)
Evaluation Requirements
- Project is sustainable after initial funding
- Project is replicable by other practices
- Provider and their appointed staff must be willing to attend the PIPA Grant Event and present their project, if asked
Evaluation Criteria
- Project addresses a problem or barrier to access or delivery of care
- Incorporates coordination or collaboration between PCP and Specialist providers (when possible)
- Innovative to practice
- Reasonable budget in relation to the scope of the project
- Reduces costs or improves quality of patient care
- Measurable outcome based on initial baseline and final data
- Commitment from the entire staff and clinic
