Medical Home Recognition

_{Since 2010, Portland IPA has assisted member clinics in obtaining Medical Home Recognition with the State of Oregon and the National Committee of Quality Assurance (NCQA) by:} 

• _{Assessing which level of recognition would be appropriate to submit for} 

• _{Developing a project plan}

• _{Providing support throughout the project}
