Thomas G. Gragnola M.D. serves as Medical Director at the Portland Interhospital Physicians Association (Portland IPA), and practices Internal Medicine at Advanced Internal Medicine (AIM) Health in Portland, Oregon. 

He attended Georgetown Medical School and completed Internal Medicine Residency at California Pacific Medical Center in San Francisco, where he served as Chief Resident in Internal Medicine.  He began his clinical and academic practice at the Providence Internal Medicine Residency Program and Ambulatory Care Clinic, where he served as an Associate Program Director and Chair of Utilization Management Committee, leading efforts to balance the challenges of teaching within a managed care environment.  After transitioning to private practice in Portland, he helped successfully guide his clinic to NCQA Medical Home recognition.  He joined AIM Health in June 2018, and maintains a busy clinical practice.  In his role as Medical Director at the Portland IPA, he lends clinical expertise and insight to the Portland IPA team as they promote numerous quality initiatives and clinical integration across a diverse spectrum of practicing clinicians in the Portland medical community.  He has been a member of the Portland IPA since 1997.
